Avoidance Coping dan Lemahnya Regulasi Diri dalam Alcohol Use Disorder: Sebuah Studi Kasus Klinis

Penelitian ini mengeksplorasi dinamika Alcohol Use Disorder (AUD) derajat berat pada dewasa awal melalui studi kasus klinis dengan pendekatan model 5P (predisposing, precipitating, perpetuating, protective, dan presenting problem). Subjek penelitian, seorang laki-laki dewasa awal, menunjukkan pola konsumsi alkohol kompulsif yang berfungsi sebagai strategi avoidance coping untuk mengurangi tekanan emosional, meskipun berdampak negatif terhadap fungsi pekerjaan, relasi sosial, dan regulasi emosi. Data dikumpulkan melalui observasi klinis, wawancara semi-terstruktur, dan sejumlah instrumen psikologis (AUDIT, WWQ, tes grafis, dan TAT). Analisis faktor predisposisi, presipitasi, dan perpetuasi mengungkap peran pola asuh neglectful, konflik interpersonal, distorsi kognitif, dan kontrol diri yang lemah dalam mempertahankan siklus penggunaan alkohol, sejalan dengan temuan terkini mengenai kontribusi adverse childhood experiences dan defisit regulasi diri terhadap risiko AUD pada dewasa muda. Hasil ini menegaskan pentingnya intervensi berbasis Cognitive Behavioral Therapy (CBT) dan pelatihan regulasi diri, yang didukung bukti efikasi dari sejumlah tinjauan sistematis dan meta-analisis terbaru, untuk meningkatkan regulasi emosi, restrukturisasi kognitif, dan pengembangan strategi coping adaptif guna mendukung pemulihan jangka panjang.
